Care Management Professional

Cape Abilities is a leading provider of adult family care services, and we are seeking a dedicated and compassionate AFC Case Manager to join our team. As an AFC Case Manager, you will play a critical role in supporting the well-being of our participants and their families.
Key Responsibilities:
- Provide case management services to support the needs of our participants and their families.
- Conduct assessments and evaluations to identify participant needs and develop individualized care plans.
- Collaborate with healthcare professionals, caregivers, and other stakeholders to ensure comprehensive support for participants.
- Monitor participant progress and make adjustments to care plans as needed.
- Provide education and training to caregivers and support staff to enhance their skills and knowledge.
- Ensure compliance with regulatory requirements and agency policies.
- Participate in quality improvement initiatives to enhance the delivery of services.
Requirements:
- Bachelor's degree.
- Social worker license from the Massachusetts Board of Registration in Social Work.
- Minimum 2 years prior experience working with elderly or disabled adults.
- Bachelor's degree plus 2 years clinical experience in the care of elderly or disabled individuals may be substituted for the above requirements.
- Attention to detail.
- Valid driver's license in the state of Massachusetts required.
- Successful completion of all state and Agency mandated trainings required upon hire.
Estimated Salary: $50,000 - $60,000 per year
