Job Summary

We are seeking a highly skilled and compassionate Skilled Nursing Care Coordinator to join our team at Traditions Health. As a key member of our interdisciplinary team, you will provide high-quality nursing care to clients of all ages in their place of residence.

Key Responsibilities
  • Administer skilled nursing care to clients in their homes, coordinating care with the interdisciplinary team, patients, and their families.
  • Assume responsibility for the coordination of care, ensuring continuity and quality of care.
  • Develop, initiate, and revise plans of care as necessary to ensure quality and continuity of care.
  • Provide education and counseling to patients and their families on meeting nursing and related needs.
  • Supervise, teach, and provide clinical direction to other nursing personnel, including home health aides and LPNs/LVNs.
  • Perform on-call responsibilities and provide on-call services to patients and their families as assigned.
Requirements
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current State license as a Registered Nurse and current Driver's License.
  • One year of experience as a Registered Nurse in a clinical care setting, with home health experience preferred.
  • Nursing skills as defined by generally accepted standards of practice.
  • Good interpersonal skills and ability to work a flexible schedule.
Working Conditions

Works in patients' homes in various conditions, with possible exposure to blood and bodily fluids and infectious diseases. Must have the ability to work a flexible schedule and travel locally.

Physical and Mental Effort

Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs.
