At Rogerson Communities, we are seeking a compassionate and dedicated Care Partner/ CNA HHA PCA to join our team. As a Care Partner, you will play a vital role in providing exceptional care and support to our residents.
Responsibilities
- Assist residents with daily living activities, including toileting, bathing, dressing, and feeding.
- Provide mobility assistance, including ambulation and use of mobility devices such as canes, walkers, and wheelchairs.
- Perform vital signs, weights, temperatures, skin care, and range of motion exercises as required.
- Assist with medication administration under the supervision of the Wellness Director.
- Document resident information and refer to pertinent lists of record keeping systems.
- Collaborate with the multi-disciplinary team to develop, implement, and evaluate care plans for residents.
- Escort residents to medical appointments within the community as needed.
- Maintain day-to-day cleanliness in resident rooms and bathrooms, and ensure tidiness in all accessible storage areas and common areas.
- Report any unsanitary conditions, fire or safety hazards, and inform the Clinical Care Coordinator of any faulty equipment needing replacement.
- Observe and report any changes in resident status to the Wellness Director and/or Clinical Care Coordinator.
Requirements
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) or Personal Care Assistant (PCA) certification.
- Knowledge of and/or 1 year of experience working with people with Alzheimer's Disease and/or related disorders.
- Ability to be flexible with scheduling and work assignments, as well as the ability to work as a team.
