At Rogerson Communities, we are seeking a compassionate and dedicated Care Partner/ CNA HHA PCA to join our team. As a Care Partner, you will play a vital role in providing exceptional care and support to our residents.

Responsibilities
  • Assist residents with daily living activities, including toileting, bathing, dressing, and feeding.
  • Provide mobility assistance, including ambulation and use of mobility devices such as canes, walkers, and wheelchairs.
  • Perform vital signs, weights, temperatures, skin care, and range of motion exercises as required.
  • Assist with medication administration under the supervision of the Wellness Director.
  • Document resident information and refer to pertinent lists of record keeping systems.
  • Collaborate with the multi-disciplinary team to develop, implement, and evaluate care plans for residents.
  • Escort residents to medical appointments within the community as needed.
  • Maintain day-to-day cleanliness in resident rooms and bathrooms, and ensure tidiness in all accessible storage areas and common areas.
  • Report any unsanitary conditions, fire or safety hazards, and inform the Clinical Care Coordinator of any faulty equipment needing replacement.
  • Observe and report any changes in resident status to the Wellness Director and/or Clinical Care Coordinator.


Requirements
  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) or Personal Care Assistant (PCA) certification.
  • Knowledge of and/or 1 year of experience working with people with Alzheimer's Disease and/or related disorders.
  • Ability to be flexible with scheduling and work assignments, as well as the ability to work as a team.
