Nurse Care Specialist

Aveanna is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Nurse. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement individualized care plans.
Key Responsibilities:- Deliver and coordinate quality patient care in accordance with physician orders and Aveanna policies.
- Conduct ongoing assessments and evaluations to identify patient needs and develop effective care plans.
- Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Document all patient interactions, assessments, and care activities in a timely and accurate manner.
- Stay up-to-date with industry developments and best practices through ongoing education and training.
- Current, unrestricted state license as a Registered Nurse.
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Ability to speak, write, read, and understand English.
- Ability to travel.
- Ability to lift 50 pounds.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Ability to sit and climb stairs.
- Visual and hearing acuity.
- Strong sense of smell and touch.
- Ability to sufficiently reposition patients and move equipment without assistance.
- Ability to respond physically and mentally to emergency situations in the home or during transport.
