Qualifications:
- Licensed Rehabilitative Specialist
- Open to candidates of all experience levels
Key Responsibilities:
- Conduct thorough assessments of patients with cognitive and/or activities of daily living (ADL) challenges, creating tailored care plans that adhere to clinical standards.
- Utilize standardized evaluation tools and evidence-based methodologies to inform treatment strategies.
- Deliver essential rehabilitative interventions to patients facing cognitive and/or ADL impairments, in line with regulatory requirements.
- Implement various therapeutic procedures as part of rehabilitation, including ADL retraining, therapeutic activities, and the use of assistive devices.
- Monitor patient progress and adjust treatment plans as necessary, ensuring compliance with clinical documentation standards.
- Evaluate treatment outcomes and modify services based on patient assessments.
- Provide guidance and support to patients, families, and caregivers regarding physical health challenges.
- Maintain effective communication with supervisors and healthcare team members about patient developments and treatment strategies.
- Compile comprehensive discharge summaries in accordance with clinical requirements.
