Qualifications:

Key Responsibilities:

  • Conduct thorough assessments of patients with cognitive and/or activities of daily living (ADL) challenges, creating tailored care plans that adhere to clinical standards.
  • Utilize standardized evaluation tools and evidence-based methodologies to inform treatment strategies.
  • Deliver essential rehabilitative interventions to patients facing cognitive and/or ADL impairments, in line with regulatory requirements.
  • Implement various therapeutic procedures as part of rehabilitation, including ADL retraining, therapeutic activities, and the use of assistive devices.
  • Monitor patient progress and adjust treatment plans as necessary, ensuring compliance with clinical documentation standards.
  • Evaluate treatment outcomes and modify services based on patient assessments.
  • Provide guidance and support to patients, families, and caregivers regarding physical health challenges.
  • Maintain effective communication with supervisors and healthcare team members about patient developments and treatment strategies.
  • Compile comprehensive discharge summaries in accordance with clinical requirements.
