SSM Health is seeking a skilled Behavioral Health Social Worker to join our inpatient team. As a key member of our multidisciplinary care team, you will provide social work assistance to patients and families, addressing social determinants of health and promoting optimal outcomes.
Key Responsibilities- Conduct psychosocial assessments of admitted patients to identify social needs and develop effective care plans.
- Collaborate with the care team to ensure proper and safe care and dispositions for patients.
- Provide therapeutic care to patients interested in social work services.
- Develop and maintain working relationships with community health, welfare, and social agencies to support patient needs.
- Participate in quality improvement initiatives to enhance standard work and patient care.
- Master's degree in Social Work.
- No experience required, but acute hospital or outpatient discharge planning experience preferred.
- Medical trauma experience preferred.
