Job Summary

We are seeking a highly skilled and dedicated Pharmacist to join our team at John Muir Health. As a key member of our pharmacy team, you will play a vital role in ensuring the effective use of medications and providing exceptional patient care.

Responsibilities
  • Verify medication orders and assess for optimal drug product selection, dosage regimen design, and effective administration.
  • Interact with patients, family, and other healthcare providers to ensure continuity of services and achieve desired patient outcomes.
  • Supervise pharmacy technicians and assume essential responsibilities of pharmacy management in their absence.
  • Lead pharmacy services in a clinical or operational area of the health system and provide expertise and oversight to staff.
  • For Ambulatory Care Pharmacists: Work with patients and other healthcare providers to ensure safe and effective medication use, complete comprehensive medication reviews, and adjust and order medications in alignment with collaborative practice agreements.
Requirements
  • Completion of an ASHP Accredited General Practice Residency (PGY1) is preferred.
  • Experience in healthcare, acute care, or a related field is preferred.
  • Completion of a Pharmacy - American Society of Healthy-System Pharmacists (ASHP) - Accredited Residency - PGY1 is preferred.
  • For Informatics (IT) Pharmacy only: ASHP Accredited residency in Information Technology is required, and a minimum of two years of clinical experience in a hospital setting or recent experience with hospital clinical information technology and pharmacy informatics knowledge is required.
  • For Oncology Pharmacy only: Experience in hematology/oncology, a basic knowledge of solid tumors, hematology, and common treatments, and the ability to educate, monitor, order, and follow labs for patients on chemotherapy regimens are strongly preferred.
  • For Pediatric Pharmacy only: Completion of a PGY-1 residency with pediatric and neonatal pharmacy services as part of the residency training or prior experience in the area of pediatric and neonatal pharmacy services is strongly preferred.
  • For Critical Care Pharmacy only: Completion of ASHP Accredited General Practice Residency (PGY1) and ASHP Accredited Critical Care Residency (PGY2) or Critical Care experience, Critical Care Board certification (BCCCP) or Critical Care experience, and a strong knowledge of critical care medicine and therapeutics are strongly preferred.
  • For Ambulatory Care Pharmacy only: A basic knowledge of chronic disease states and common treatments, the ability to educate, monitor, order, and follow labs for medications related to disease state management, and prior experience in AmCare Clinic setting and disease state management are strongly preferred.
Certifications/Licensures
  • RPH Registered Pharmacist - California Board of Pharmacy is required.
  • BLS Basic Life Support - American Heart Association is required.
  • ACLS Advanced Cardiac Life Support - American Heart Association is preferred and required by 3/31/20 for staff working Unit Based Shifts.
  • PALS Pediatric Advanced Life Support - American Heart Association is required for staff working in Pediatric Unit Based shifts and Emergency Dept Unit Based Shifts.
  • APh Advanced Practice Pharmacist License is preferred.
Work Schedule

Wednesday-Saturday, 10.0 - 07:30 - 18:30 No Waive (United States of America)

Pay Range

$84.64 - $114.26 Hourly
