As a Registered Nurse at Rochester Manor, you will have the opportunity to make a significant impact on the lives of our patients and residents. Position Highlights:
* Conduct nursing assessments and collaborate with the nursing team, interdisciplinary staff, patients, and their families to create effective care plans.
* Set realistic, measurable short- and long-term goals for patients, implementing interventions as necessary to reduce rehospitalizations and continuously evaluate outcomes.
* Delegate patient care tasks to staff and oversee their execution to ensure compliance with care protocols.
* Communicate pertinent patient information to assigned staff and facilitate smooth transitions between shifts.
Qualifications:* Must be a graduate of an accredited nursing program and hold a current license from the State Board of Nursing.
