About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team in providing advanced illness management and palliative care services to our patients. As a key member of our clinical team, you will work collaboratively with our physicians and other healthcare professionals to deliver high-quality, patient-centered care.

Responsibilities
  • Assess and manage patients with advanced illnesses, providing comprehensive care and support to patients and their families.
  • Develop and implement individualized treatment plans, working closely with our interdisciplinary team to ensure seamless care coordination.
  • Prescribe medications, order diagnostic tests, and provide ongoing care and support to patients in their homes or in our facilities.
  • Communicate effectively with patients, families, and healthcare teams to ensure that care is coordinated and patient-centered.
  • Participate in ongoing education and professional development to stay current with best practices in palliative care and advanced illness management.
Requirements
  • Master's degree in Nursing from an accredited program.
  • Current licensure as a Registered Nurse and Nurse Practitioner in the state of practice.
  • Minimum of two years of experience as a Nurse Practitioner in a palliative care or advanced illness management setting.
  • Strong leadership and communication skills, with the ability to work effectively in a team environment.
  • Knowledge of palliative care principles, including pain and symptom management, and experience with electronic medical records.
