Job Title: Home Health Speech Language Pathologist

Job Summary:
VNA Health Group is seeking a skilled Home Health Speech Language Pathologist to join our team. As a Home Health Speech Language Pathologist, you will be responsible for providing speech and language therapy evaluation and treatment to patients in their homes or alternate community settings.

Responsibilities:
  • Assesses and evaluates client for speech and language therapy needs and develops and implements appropriate plan of action in collaboration with the physician
  • Documents all changes in client's condition and updates care plans and outcomes accordingly
  • Identifies counseling needs and plans appropriate teaching strategies
  • Monitors quality and appropriateness of services provided
  • Adheres to agency infection control and safety policies
  • Adheres to the organization's policy in regards to absenteeism and appearance

Qualifications:
  • Licensed Speech-Language pathologist with CCC-SLP certification and Master's degree in Speech-Language Pathology
  • 2-3 years of prior clinical experience and community based experience preferred
  • Strong computer skills
  • Strong communication and interpersonal skills
  • Valid NJ drivers license

Working Conditions/Physical Demand :
Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
