Job Overview:
St. Elizabeth Healthcare is seeking a highly skilled Health Transition Coordinator to join our team. As a key member of our interdisciplinary team, you will be responsible for coordinating patient transitions between care settings, ensuring seamless and efficient care delivery. Your primary responsibilities will include assessing patient needs, developing effective care plans, and communicating with patients, families, and healthcare professionals.

The ideal candidate will possess strong interpersonal skills, with the ability to build rapport with diverse stakeholders. A graduate degree in nursing or a related field is required, along with licensure as an RN in the state where work is being performed.
