Care Coordinator Specialist

As a Care Coordinator Specialist at St. Elizabeth Healthcare, you will play a vital role in ensuring patients receive the best possible care throughout their continuum of treatment. In collaboration with interdisciplinary teams, you will identify and manage patient flow to optimize continuity of care.
The ideal candidate will possess strong organizational skills, attention to detail, and excellent communication abilities. A graduate degree in nursing or a related field is required, along with licensure as an RN in the state where work is being performed.
