Job Title: Care Manager Supervisor

Job Summary: We are seeking a highly skilled Care Manager Supervisor to join our team at SCO Family of Services. The successful candidate will provide administrative oversight to a team of Care Managers, ensuring seamless coordination and delivery of care to children, adults, and families with complex needs.

Key Responsibilities:

  • Oversee the identification of eligible Health Home members and provide administrative field supervision to a unit or up to 8 individual Care Managers.
  • Develop and nurture leadership qualities in all line staff, addressing administrative issues and monitoring required documentation.
  • Supervise Care Managers, providing constructive feedback, and participating in forums, meetings, and conference calls as needed.
  • Ensure compliance with governing bodies' requirements and facilitate case discussions with Care Managers in the field.
  • Provide adequate compliance support, review electronic case records, and maintain accurate personnel files.
  • Lead and direct the team, setting priorities, and coordinating plans for audit preparation.
  • Ensure timely completion of tasks, maintain accurate records, and report to Directors as needed.

Requirements:

  • Master's degree preferred; B.A. in Social Work or related field with 4+ years of experience in child welfare or a related field.
  • Knowledge of best practices in health, preventive, and wellness care, with a passion for advocating on behalf of staff, children, and families.
  • Excellent communication and leadership skills, with the ability to remain calm in crisis situations and maintain confidentiality.
  • Strong team player with a trusting and supportive relationship with subordinates, and sensitivity to cultural differences among staff and clients.
