Job Summary

The Care Management Supervisor will provide administrative oversight to a team of Care Managers who coordinate care for children, adults, and families in the Care Management Program. The Care Manager plays a critical role in achieving the overall goals for coordination, integration, and partnership in the delivery of care to individuals with complex needs.

Key Responsibilities

  • Oversee the identification of eligible Health Home members and the program enrollment and consent process.
  • Provide administrative field supervision to Care Managers and develop leadership qualities in staff.
  • Monitor and review Care Management documentation and track cases to ensure compliance with governing bodies.
  • Supervise staff and provide performance feedback, including recognition, rewards, and disciplinary action as needed.
  • Coordinate and ensure execution of plans for audit preparation and review electronic Health records.
  • Provide information regarding audit readiness, including Key Performance Indicators (KPI) data and case records.

Qualifications

  • Master's degree preferred, with a B.A. in Social Work or a related field and 4+ years of experience in child welfare.
  • Knowledge of best practices working with children and families in health, preventive, and wellness care.
  • Passion and knowledge for advocating on behalf of staff, children, and families.

  • Maintain workplace boundaries and promote a productive and positive work atmosphere.
  • Uphold agency/program vision and mission in interactions with colleagues and families served.
  • Adhere to agency Code of Conduct.

  • Work well with others and provide direction and guidance to subordinates.
  • Evidence of the ability to practice a high level of confidentiality.
  • Develop a trusting and supportive relationship with subordinates.
  • Have a strong sensitivity to cultural differences presented among staff and clients within the agency.
  • Demonstrate excellent crisis intervention skills and remain calm in crisis situations.
