About the Role:

  • Deliver Quality Patient Care: Provide skilled nursing care to patients in their homes, adhering to physician orders and best practices.
  • Collaborate with Multidisciplinary Teams: Work closely with healthcare professionals to ensure seamless care coordination and effective communication.
  • Assess and Document Patient Needs: Continuously observe and assess patient condition and care needs, documenting all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Participate in Nursing Care Planning: Collaborate with the nursing team to develop, implement, and update individualized care plans.

Requirements:

  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.

Preferred Qualifications:

  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.

Physical Requirements:

  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.

Estimated Salary: $29.00 per hour
