Nurse Practitioner

BAYADA Home Health Care, a leading provider of home health care services, is seeking a skilled and compassionate Nurse Practitioner to join our team as a Home Health Care Specialist.

As a Nurse Practitioner with BAYADA, you will have the opportunity to work in a variety of settings, including private homes, nursing facilities, and assisted living communities. You will provide high-quality, patient-centered care to individuals of all ages, from neonates to geriatrics, with a focus on delivering evidence-based practice and promoting positive outcomes.

You will work closely with our interdisciplinary team to develop and implement individualized care plans that address the unique needs and goals of each patient. Your expertise and dedication will make a meaningful difference in the lives of our clients and their families.

• Conduct comprehensive assessments of patients' physical, emotional, and social needs
• Develop and implement individualized care plans that address patients' goals and preferences
• Provide high-quality, patient-centered care, including medication management, wound care, and education
• Collaborate with other healthcare professionals to ensure seamless transitions of care
• Document all patient interactions and outcomes in accordance with regulatory requirements

To be successful in this role, you must have:

• A Master's degree in Nursing or a related field
• A valid license to practice as a Nurse Practitioner in the United States
• At least 1 year of experience in home health care or a related setting
• Excellent communication and interpersonal skills
• A commitment to delivering high-quality, patient-centered care

BAYADA Home Health Care offers a competitive compensation package, including:

• An estimated salary range of $85,000 - $110,000 per year, based on experience and location
