Job Title: Registered Nurse Field Supervisor

VNS Health is seeking a skilled Registered Nurse to serve as a Field Supervisor for our Home Health team. As a key member of our team, you will be responsible for providing clinical guidance and support to our Home Health Aides, ensuring that our patients receive high-quality care and exceptional outcomes.

Key Responsibilities:
  • Conduct initial and ongoing supervisory field visits to patients' homes to assess their needs and provide training support.
  • Document patient visits and care plans in our electronic medical record (EMR) system.
  • Orient and supervise Home Health Aides in the field, ensuring they have the necessary skills and knowledge to provide quality care.
  • Communicate effectively with our clinical care team to ensure seamless care coordination and timely interventions.
  • Collaborate with our team to develop and implement individualized care plans that meet the unique needs of each patient.
