Position: Licensed Practical Nurse
The role of a Licensed Practical Nurse involves a significant responsibility in delivering nursing care and support to residents. This position operates under the direct oversight of a Charge Nurse, with potential supervisory duties over Nursing Assistants and support staff.
Key Responsibilities:
- Facilitates the transition of new residents into the facility, ensuring they understand the mission and objectives of Willow Point Rehabilitation and Nursing Home.
- Reports any incidents or changes in resident conditions to the Charge Nurse and accurately documents these in the resident's medical records.
- Engages in discussions regarding resident care with the Charge Nurse to address clinical concerns.
- Collaborates with an interdisciplinary team to assist in the development, execution, and ongoing assessment of individualized care plans.
- Monitors, documents, and communicates any changes in residents' health status.
- Updates care plans as necessary to reflect changes in resident needs.
- Administers therapeutic treatments as prescribed.
- Dispenses medications (excluding I.V. medications) to residents, monitoring for side effects and effectiveness.
- Ensures proper management and accountability of controlled substances.
- Keeps comprehensive nursing care and administrative records.
- Participates in staff meetings and continuing education programs.
Essential Knowledge, Skills, and Abilities:
- Proficient understanding of contemporary nursing practices.
- Competence in applying practical nursing techniques and procedures for resident care.
- Effective communication skills, both verbal and written.
- Ability to foster and maintain collaborative working relationships.
- Demonstrates reliability, resourcefulness, keen observational skills, and professionalism.
- Capable of operating medical equipment such as suction machines and oxygen units.
- Ability to maintain meticulous records and generate reports.
- Physical fitness appropriate for the demands of the role.
Minimum Qualifications:
- Completion of a nursing program accredited by the New York State Education Department, qualifying for Licensed Practical Nurse licensure in New York State.
- Possession of a limited permit to practice as a Licensed Practical Nurse in New York State, with licensure required within six months of appointment.
Exemptions:
- Graduates from New York State nursing programs who have applied for licensure may work under the supervision of a registered nurse for a limited period following graduation.
