Job Description

Pleasant Acres Skilled Nursing and Rehabilitation Center is seeking a compassionate and experienced Social Services Director to join our team. As a key member of our interdisciplinary team, you will be responsible for conducting comprehensive assessments of residents' psychosocial needs, developing and implementing individualized social service plans, and advocating for residents and their families.

Key Responsibilities:

  • Conduct comprehensive assessments of residents' psychosocial needs and preferences.
  • Develop, implement, and evaluate individualized social service plans in collaboration with interdisciplinary teams.
  • Act as an advocate for residents and their families, addressing concerns and facilitating access to services and support.
  • Connect residents and families with community resources, support services, and financial assistance programs.
  • Provide individual and group counseling for residents and families to address emotional, social, and behavioral issues.
  • Ensure compliance with state and federal regulations regarding social services in long-term care settings.
  • Supervise and train social services staff, fostering a supportive and effective work environment.

Requirements:

  • Bachelor's degree in social work, psychology, sociology, or a related field is required; a Master's degree is preferred.
  • Must hold a valid license as a Social Worker (LBSW or LISW) in the state of Iowa.
  • Previous experience in a social services role within a healthcare or long-term care setting is preferred.
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and interdisciplinary teams.
