Job Summary

We are seeking a skilled Nurse Care Specialist to join our team at SKLD Leonard. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care to our residents.

Key Responsibilities
  • Assess residents' conditions and develop individualized care plans to ensure their physical and emotional needs are met.
  • Provide preventative, supportive, maintenance, and rehabilitative care to residents, including administering medications, performing treatments, and monitoring vital signs.
  • Collaborate with the Director of Nursing, Unit Managers, and other healthcare professionals to ensure seamless care delivery and achieve optimal resident outcomes.
  • Document resident observations, interventions, and outcomes in a timely and accurate manner.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance resident care.
Requirements
  • Current state license as a Registered Nurse or Licensed Practical Nurse.
  • CPR certification.
  • One year of professional nursing experience in the long-term care setting preferred.
  • Knowledge of applicable state nursing regulations and current federal laws.
Preferred Qualifications
  • Strong organizational and time management skills.
  • Ability to apply mathematical concepts to practical situations.
  • Basic computer knowledge.
