Job Title: Case Manager (Homeless Services)

Location: Inglewood, CA

Salary: $21.00 - $26.00 per hour

Our goal is to provide full range support services that lead to sustainable housing solutions and improved quality of life. We are looking for a compassionate Case Manager to join our team to impactful work in the community.

Summary:

The Case Manager will manage personalized support to individuals and families experiencing homelessness. You will work directly with homeless clients to manage intakes, individualized service plans, and connect them with the resources and services.

Key Responsibilities:

  • Client Intake: Conduct assessments to understand clients' needs for housing stability. Develop individualized service plans.

  • Resource Coordination: Assist clients in accessing services, including emergency shelters, transitional housing, financial assistance, job training, and healthcare.

  • Case Management: Maintain contact with clients to monitor progress and work on adjustments to service plans.

  • Documentation: Maintain accurate and confidential records of client interactions.

Qualifications:

  • Experience: At least 1-2 years of experience in case management, social services, or a related field.

  • Skills: Strong interpersonal and communication skills.

  • Empathy: Demonstrated compassion and a non-judgmental approach to working with vulnerable populations.

  • Technology: Proficiency with Microsoft Office Suite and experience using case management software or databases.
