Job Title: Success Coach

Families & Youth Inc is seeking a highly motivated and compassionate Success Coach to join our team. As a Success Coach, you will play a vital role in supporting families in need, helping them overcome barriers and achieve maximum wellness.

Key Responsibilities:
  • Coordinate services between providers and clients to ensure seamless support.
  • Interpret agency services to clients and the community, providing clear and concise information.
  • Maintain accurate and up-to-date individual case records for clients receiving services.
  • Provide specialized case management services to meet the unique needs of each client.
  • Educate clients on available community resources and services, empowering them to make informed decisions.
  • Develop and implement programs and services to benefit clients and the community.
  • Attend relevant community meetings to stay informed and build partnerships.
  • Provide intensive home-based individual and family client services, adhering to CYFD Placement Prevention Services Contract standards.
  • Meet with families at least weekly throughout the duration of the case, ensuring consistent support and progress.
  • Be accessible to families 24/7, providing timely support and guidance.
  • Conduct regular face-to-face visits with clients, fostering strong relationships and trust.
  • Collect, evaluate, and report data to measure the effectiveness of CYFD contract activities.
  • Provide weekly reports to referral agencies and other service providers, ensuring seamless communication.
  • Participate in case staffing/conferences as needed, collaborating with referring agencies and other service providers.
  • Deliver parenting education curriculum to families in their homes, promoting healthy relationships and well-being.
Qualifications and Experience:
  • Bachelor's Degree in Social Work, Education, Counseling, Psychology, Sociology, Criminal Justice, Family Studies, or a related field approved by CYFD.
  • Two years of experience working with communities on health or social service-related matters, social work/case management experience, behavioral health, and/or healthcare.
  • Level II Success Coach Qualifications: Bachelor's Degree in Social Work from an accredited college/university and four years of experience, or a Bachelor's Degree in Education, Counseling, Psychology, Sociology, Criminal Justice, Family Services, or a related field approved by CYFD and six years of experience.
