Job Summary:

Angels of Care Pediatric Home Health is seeking a skilled Speech-Language Pathologist to join our team. As a Speech-Language Pathologist, you will be responsible for implementing treatment programs to assist pediatric patients with cognitive, speech, language, feeding/swallowing and/or social/emotional disabilities and delays.

Responsibilities:

Conduct comprehensive evaluations and assessments to identify patient needs and develop individualized treatment plans.

Implement speech therapy services in the home and community, utilizing evidence-based practices and techniques to maximize patient outcomes.

Collaborate with pediatric patients, families, and caregivers to educate and empower them to develop skills to carry over therapeutic activities into daily routines.

Participate in care coordination and communication with referral partners to ensure seamless service delivery.

Maintain accurate and timely documentation of patient care services and care coordination in our electronic medical record system.

Requirements:

ASHA certification
State License
Current CPR certification
A minimum of 1 year of experience preferred
