About This Job

We are seeking a Hospice MSW to provide home-based social services to patients dealing with end-of-life issues. As a Hospice Medical Social Worker for Interim HealthCare, you will visit patients and families in their homes, helping them access resources, counseling, and support needed during challenging times.

The Role

In this role, you will:

  • Provide home-based social services to patients facing end-of-life issues, including care management, pain, symptoms associated with chronic and terminal illnesses, depression, anxiety, poor nutrition or lack of resources that make managing their journey difficult.
  • Work as part of a hospice team, which includes an RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT, OT, or SLP, focused on the patient's plan of care and individual goals.
  • Monitor a patient's psychosocial condition and identify social and emotional needs.
  • Conduct patient assessments, document progress, and ensure patients are moving toward goals.
  • Provide counseling, community resource planning, crisis intervention, and advocacy.
  • Consult with family and caregivers on patient's plan of care and how to help them progress.

Requirements

To be successful in this role, you must have:

  • A graduate degree in social work from a master's program accredited by the Council on Social Work Education.
  • An active license to practice as a Social Worker in the state where employed, with a valid LMSW or LLMSW in good standing.
  • A minimum of one year of experience in healthcare, preferably in hospice or homecare settings.
  • Reliable transportation, including a valid driver's license and car insurance.

Estimated Salary Range: $55,000 - $75,000 per year, depending on location and experience.
