We are seeking a skilled Occupational Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Occupational Therapist, you will provide high-quality care to patients in their homes, helping them to regain independence and improve their quality of life.

About the Job

This is a full-time position, working Monday to Friday, 8:15AM to 5:00PM. You will be responsible for providing skilled occupational therapy services to patients in their homes, as ordered by the attending physician. Your duties will include:

  • Providing comprehensive skilled services as ordered by the attending physician
  • Observing, recording, and reporting patient reactions, changes, and progress to the attending physician and any appropriate agency staff
  • Developing an individualized OT plan of care and making adjustments as needs change
  • Using activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity
  • Helping to decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions
  • Counseling patient and family on the disease/injury and how to manage in all phases of occupational therapy
Requirements

To be successful in this role, you will need:

  • A current license in the state of employment as an Occupational Therapist
  • Minimum one-year experience as a OT in an acute care setting. Home care experience preferred
  • A valid driver's license and auto insurance in your name as a driver
  • The ability to perform all physical demands of the job
