Job Description

This position involves delivering care in accordance with patient care plans, policies, and procedures of the organization and principles of relationship-based care. You will participate as a member of the healthcare team and accept delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family.

  • You will collect data through observation and communicate information to assist the RN and/or provider in patient assessment and care planning.
  • You will deliver routine care for patients and observe patients' health.
  • You will interview patients and record their medical history and physical condition.
  • You will obtain patient vital signs, including pulse, blood pressure, temperature, and respiration.
Duties and Responsibilities

The duties of this position may include but are not limited to:

  1. Collecting data through observation and communicating information to assist the RN and/or provider in patient assessment and care planning.
  2. Participating as a member of the healthcare team and accepting delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family.
  3. Delivering care in accordance with patient care plans, policies, and procedures of the organization and principles of relationship-based care.
Working Conditions

Working conditions may involve exposure to infectious disease, blood, body fluids, and/or tissue as well as other unpleasant elements such as accidents, injuries, and illness.

Qualifications

To be eligible for this position, you must possess or be eligible to obtain a license to practice as a Licensed Practical Nurse in New York State.

We offer competitive pay, with an estimated annual salary range of $43,768 - $73,485 based on location and experience.
