Nurse Care Specialist

Job Summary:
As a Registered Nurse at Advocate Health, you will play a vital role in delivering high-quality patient care and contributing to a collaborative healthcare team. Your primary responsibility will be to assess patient needs, develop and implement individualized care plans, and evaluate their effectiveness.
Key Responsibilities:
- Utilize the nursing process to assess patient needs and develop comprehensive care plans.
- Collaborate with patients and families to ensure their involvement in the care planning process.
- Develop and maintain therapeutic relationships with patients and families throughout the healthcare continuum.
- Anticipate and recognize significant changes in patient condition, status, and need for care, and take appropriate actions.
- Implement strategies to reduce patient risk and increase patient safety.
- Assess patient and family readiness to learn and individualize the approach as necessary.
- Actively coordinate discharge plans utilizing an interdisciplinary approach.
- Apply evidence-based practice to deliver patient care.
- Consistently incorporate nursing and regulatory standards of care into practice.
Requirements:
- Registered Nurse license issued by the state in which you practice.
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association.
- Completion of an accredited or approved program in Nursing.
Preferred Qualifications:
- Proficient computer skills, including the use of electronic mail and electronic medical records systems.
- Excellent communication, listening, and interpersonal skills.
- Ability to develop rapport and maintain positive, professional relationships with patients, team members, and physicians.
- Initiative and ability to work collaboratively with others.
- Effective decision-making and problem-solving skills.
Working Conditions:
Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday. Must be able to lift up to 50 lbs. from floor to waist, lift up to 20 lbs. over the head, and carry up to 40 lbs. a reasonable distance. May be exposed to chemical and hazardous waste, as well as blood and body fluids, and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
