Patient Placement Specialist

We are seeking a skilled Patient Placement Specialist to join our team at Prisma Health. As a key member of our post-acute care team, you will play a critical role in evaluating, monitoring, and coordinating patient readiness and transfer for post-acute services.
Key Responsibilities- Evaluate patient needs and identify the most appropriate post-acute care services to ensure quality and cost-effective use of clinical resources.
- Coordinate with patients, physicians, and multidisciplinary care teams to develop and implement individualized care plans.
- Monitor and analyze clinical and fiscal outcomes to ensure compliance with regulatory guidelines and identify opportunities for improvement.
- Act as a resource for other staff members, providing clinical knowledge and expertise to support the delivery of high-quality patient care.
- Associate's Degree in Nursing or related field.
- 2 years of post-acute related experience.
- RN licensure and certification in a relevant specialty (e.g., CRRN).
