Overview:

We are seeking a qualified Registered Nurse with a minimum of two years of experience in geriatric care. This role offers a unique opportunity to transition from a hospital environment to a dynamic home health care setting, where you can make a significant impact on client care.

Position Summary:

The Registered Nurse Care Manager is responsible for delivering nursing services in accordance with the State Nurse Practice Act and established agency policies. This role involves reviewing caregiver qualifications, conducting comprehensive assessments, and ensuring high-quality care delivery.

Key Responsibilities:

  • Conduct thorough assessments of client health status, including physical and psychosocial factors.
  • Develop and implement individualized care plans based on assessment data and best practices.
  • Provide skilled nursing interventions aimed at achieving positive client outcomes.
  • Maintain accurate and timely documentation of client care and progress.
  • Collaborate with healthcare team members to enhance client care and outcomes.
  • Supervise and mentor caregivers, fostering their professional development.

Qualifications:

  • Graduate of an accredited nursing program.
  • Active Registered Nurse license in the State of Maryland.
  • At least one year of recent experience in home care for elderly clients.
  • Current CPR certification.
  • Excellent communication and interpersonal skills.
  • Commitment to coaching and developing caregivers in elder care.
  • Willingness to travel as required.
