About the Role

We are seeking a highly skilled and compassionate ACCS Clinician to join our team at Open Sky Community Services. As a key member of our multidisciplinary team, you will play a vital role in delivering evidence-based interventions and providing quality, compassionate care to individuals served.

Key Responsibilities
  • Develop and implement Comprehensive Assessments and Individualized Treatment Plans using Person-Centered Planning.
  • Build relationships with individuals served and develop shared understanding of Person's goals and the steps necessary to achieve goals.
  • Collaborate with applicable Care Coordination Entities, Emergency Service Providers, outpatient clinics, and other providers/entities to share information and ensure that the needs of the person are fully met.
  • Conduct individual sessions, facilitate support groups and treatment planning meetings with individuals served.
  • Provide clinical consultation and training to staff on the integrated team.
Requirements

To be successful in this role, you will need:

  • A Master's Degree in Social Services field with current licensure or license eligibility.
  • Previous experience working with adults with mental health challenges.
  • A valid driver's license and acceptable driving record.

  • A starting salary range of $60,000 - $70,000 per year, based on current licensure and experience.
