As a Registered Nurse with Elite Home Health, you will have the opportunity to work in a dynamic and supportive environment that values collaboration, innovation, and excellence. You will be responsible for providing comprehensive nursing care to patients in their homes, working closely with our interdisciplinary team to develop and implement personalized care plans.
Key Responsibilities:
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
- Develop and implement individualized care plans that address patients' unique needs and goals, working closely with patients, families, and healthcare providers to ensure seamless care coordination.
- Provide high-quality nursing care to patients in their homes, including administering medications, wound care, and other treatments as prescribed.
- Collaborate with our interdisciplinary team to ensure comprehensive care, including physical therapy, occupational therapy, and social work services.
- Document patient progress and outcomes, maintaining accurate and timely records to inform care decisions and ensure continuity of care.
Requirements:
- Current RN licensure in the state of practice
- Current CPR certification
- Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation
