Job Summary

Signify Health is seeking a skilled Nurse Practitioner to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. As a Nurse Practitioner for In-Home Visits, you will have the opportunity to work flexible hours, connect with people one-on-one, and help them re-engage with their health.

Key Responsibilities
  • Conduct in-home visits that focus on a person's overall health and well-being and identify potential barriers to care
  • Earn up to $2,500 to $4,000 each week (average 6-9 visits daily, $80 per completed assessment)
  • Take advantage of our Market Multiplier, which can increase your per-visit rate up to 160% (depending on working counties)
  • Earn additional compensation via our pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests)
  • Work flexible shifts, including Monday-Sunday (no nights/weekends required)
  • Choose from short-term or long-term contracts

Requirements
  • Master's degree in Nursing
  • Arkansas NP license (preferred)
  • Ability to relocate to Springdale, AR before starting work
