Job Overview
- The Patient Care Nursing Specialist is responsible for overseeing the coordination of clinical services for patients transitioning from hospital to home care.
- This role involves promoting, educating, and coordinating services during patient transfers to home healthcare or hospice care.
Qualifications
- A Bachelor’s degree in nursing is preferred.
- Graduation from a CCNE or NLN accredited nursing program is strongly preferred.
- A minimum of 2 years of experience in home health or hospice care is preferred.
- Familiarity with community resources is advantageous.
Licensure/Certification Requirements
- A valid Registered Nurse license in the State of South Dakota is required, with multi-state licensure eligibility.
- A Minnesota RN license may be necessary depending on the service area, to be obtained within 90 days if required.
- Integrated Care Management Model Certification is preferred.
- A valid driver’s license and insurability under Avera Health's automobile liability insurance are required.
Work Schedule
- 8:00 am - 4:30 pm as needed; PRN
Compensation
The hourly wage for this position is competitive, based on experience, with a range starting at $44.25 per hour.
