Job Summary

We are seeking a dedicated and compassionate Registered Nurse to provide high-quality care to medically fragile children and adults in their homes or schools. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Provide routine nursing care to assigned patients, including those with G-Tubes, Trachs, Vents, and CVLs.
  • Monitor and act on patient progress, identifying any changes in status.
  • Perform patient teaching and training for patients and families.
  • Communicate effectively with all staff, patients, and families.
  • Comply with all policies and procedures, upholding the expectations of Advanced Care Partners.
Requirements
  • Valid RN nursing license in Texas.
  • Active CPR certification.
  • Two or more years of experience preferred; new graduates accepted based on completion of preceptorship.
  • Valid PPD skin test.
