About the Role
As a Home Health LPN with HealthONE Healthcare at Home, you will have the opportunity to provide quality, skilled nursing care to patients in the comfort of their own homes. Our team is dedicated to delivering exceptional patient care and support, and we are seeking a compassionate and skilled LPN to join our team.
Key Responsibilities
- Administer quality, skilled nursing care to patients in the home, under the direction and supervision of a Registered Nurse and/or Director, Professional Services.
- Verify the Plan of Care prior to each visit and provide care according to the physician's orders, assessment data, established standards and guidelines.
- Perform an ongoing assessment during each visit and document data in patient medical records.
- Communicate significant findings, problems, or changes in the patient's condition to the supervising RN and/or Director of Professional Services and the physician.
Requirements
- Basic Cardiac Life Support must be obtained within 30 days of employment start date.
- Licensed Practical / Vocational Nurse (LPN / LVN) certification.
- Drivers License.
- Vocational School Graduate.
