About Us:
Care Advantage specializes in delivering personalized home care and companionship services, whether in a home, hospital, nursing facility, or rehabilitation center. We customize care plans to meet the unique needs of our clients, whether they are transitioning from a hospital setting or require companionship for a few hours each day.

A DAY IN THE LIFE OF A Home Care Supervisor RN/ Admissions RN

  • Initiates contact with patients to explain available services
  • Conducts start-of-care assessments and documentation for new patients
  • Creates tailored care plans for each individual patient
  • Guides assigned care team members on patient needs and care plan specifics
  • Communicates patient progress to relevant parties
  • Maintains ongoing relationships with clients, advocating for their needs
  • Builds connections with community partners and referral sources
  • Keeps Clinical Leadership and Office Administrator informed of patient conditions and medical updates
  • Reviews documentation submitted by caregivers
  • Participates in interdisciplinary team meetings to ensure comprehensive patient care
  • Performs additional duties as assigned by the Administrator

Qualifications:

  • Must hold an active Virginia Registered Nurse license
  • At least 1 year of experience in a nursing role (LPN to RN acceptable)
  • Minimum of an Associate's Degree in Nursing
  • Comfortable collaborating within cross-functional teams
  • Effective verbal and written communication skills across all organizational levels
  • Strong presentation abilities
  • Reliable transportation for home visits with mileage reimbursement
  • Proficient in computer/iPad technology for documentation purposes
