Community Integration Specialist

We are seeking a dedicated and compassionate Community Integration Specialist to join our team at Lakeview Specialty Hospital & Rehab.
As a Community Integration Specialist, you will play a vital role in providing assistance and treatment to program participants, ensuring their active participation in the rehabilitation program, and participating as a member of the interdisciplinary treatment team.
Key Responsibilities:
- Function as a therapeutic agent by demonstrating teaching, reinforcing, anticipating, interacting, and noticing program participant behavior.
- Tend to the program participant's immediate needs for safety and perform all aspects of participant care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Provide treatment in accordance with unit and facility policies through active participation with residents during activities, ADL's, school, work, meals, groups, etc., in a way that fosters independent functioning on the part of the resident.
- Display age-appropriate interaction skills and actively participate in exchange of information, interacting with family members and others in a professional manner.
- Demonstrate consistent and reliable documentation on Education forms in accordance with Individual Service Plan (ISP) and prescribed treatment interventions.
- Document changes and/or unusual behaviors/situations and complete other unit paperwork (behavior plans, checklists, etc.) as required.
- Demonstrate flexibility and responsibility in scheduling, coverage, and provision of clinical services to meet the needs of the residents and families.
- Participate in all assigned training programs to meet state and company requirements and adhere to the administrative, clinical, and human resource policies of Lakeview Specialty Hospital & Rehab.
Qualifications:
1. Skill, Knowledge, and Abilities: Knowledge of the sequela of brain injury and other neurological disorders, knowledge of physical, cognitive, psychological, emotional, and/or communication disabilities, First Aid course, Standard Precautions, CPR certified required.
2. Education: Required: High school diploma or equivalent may include completion of certified nursing assistant course with certificate. Preferred: Degree in the area of Social Services. Certified as CNA if CNA.
3. Licensure: Valid driver's license and good driving record preferred.
4. Preferred at least one year of experience working within a residential based or behavioral program.
