Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Harmony Pointe Nursing Center. As a LPN, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Key Responsibilities

Assess patients by physical examination, including pertinent diagnostic testing to determine health status.

Administer medications and treatments as ordered by physicians.

Participate in the care planning process to develop and implement individualized care plans for each resident.

Supervise LPNs and certified nursing assistants to ensure the delivery of high-quality patient care.

Communicate effectively with physicians, residents, and family members regarding changes in resident conditions, diagnostic test results, and other relevant information.

Document assessments and care in compliance with standards of care and company policy.

Requirements

Graduate of an accredited school of nursing with a current, unencumbered LPN license in Colorado.

Current CPR certification.

One year of experience in a long-term care environment preferred.
