At Community Health Network, we are seeking a compassionate and skilled Hospice Nurse to join our team. As a Hospice Nurse, you will play a vital role in providing high-quality care to patients and their families.
Job SummaryThe Hospice Nurse will be responsible for providing comprehensive care to patients with terminal illnesses, including pain management, symptom control, and emotional support. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Responsibilities- Provide direct patient care, including administering medications, wound care, and other treatments as needed.
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs.
- Develop and implement individualized care plans to meet patients' unique needs.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Provide emotional support and education to patients and their families.
- Graduate of an accredited nursing program.
- Current RN licensure in the state of Indiana.
- At least one year of experience in hospice or palliative care.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
