About the Role

The Hospice LPN works closely with RN Case managers to meet the needs of hospice patients and their families in their home and in long-term care facilities. You'll demonstrate performance consistent with professional standards of practice, care, performance, the Nurse Practice Act, and the mission and goals of the organization.

Responsibilities
  • Provide patient care in the North region, including Indianapolis and Anderson, IN.
  • Manage patient care in the healthcare continuum to achieve optimum outcomes in a safe and cost-effective manner.
  • Work collaboratively with the interdisciplinary team to develop and implement patient care plans.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of a recognized accredited school of practical nursing or non-accredited school of practical nursing with three years related professional nursing experience.
  • Licensed as a Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency.
  • Current Indiana Driver's License.
