About the Job
This position is responsible for providing skilled nursing care to patients in their homes, under the supervision of a registered nurse. You will work closely with our multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities
- Provide one-on-one patient care in a home environment
- Execute physicians' prescribed plans of care and document all activities, assessments, and nursing actions in a timely manner
- Administer prescribed medication, treatments, and therapies as ordered by the physician
- Conduct patient assessments, coordinate care, and provide education to patients and their families on disease management, self-care techniques, and prevention strategies
Requirements
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a licensed nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
