Community Health Advocate

In this role, you will be responsible for coordinating care for clients, developing and implementing care plans, providing outreach and assessment services, and collaborating with a multidisciplinary team to facilitate client engagement and address barriers to service utilization.
Key Responsibilities:

Requirements: A Master's degree in a human services field with one year of experience, or a bachelor's degree with two years of experience, or CASAC with two years of experience. Alternatively, a bachelor's degree with three years of experience or two years as a Health Home care manager for the Severe Mental Illness (SMI) or serious emotional disturbance (SED) population.
