Job Title: Home Visitor

Job Summary:

NATIVE HEALTH is seeking a skilled Home Visitor to provide developmental information, support, and encouragement to individuals and families, with a primary focus on pregnant women, infants, young children, and expectant fathers. The successful candidate will utilize the Parents As Teachers model to deliver case management services and build trust and rapport with clients and their family members.

Responsibilities:

  • Service Delivery: Provide coordination and delivery of services to enable healthy and well-adjusted childhood development, conducting personal in-home visits and attending required trainings.
  • Family Enrollment: Identify and enroll families with children under the age of eight into the program, fostering familiarity with resources and actively recruiting pregnant and postpartum women through outreach events and community partnerships.
  • Group Activity/Class Development: Engage in the organization and preparation of monthly events focusing on child development, parent-child interaction, and education on family well-being.
  • Documentation & Record-Keeping: Document and keep records of all visits and case management services provided to families, maintaining strict client confidentiality and adhering to HIPAA regulations.

Qualifications:

  • Bachelor's degree in early childhood development/education, social work, public health, or related discipline.
  • At least two years' experience serving or working within American Indian/Alaskan Native population.

Standard Employment Requirements:

  • Valid and unrestricted Arizona driver's license with no DUI/DWI or reckless driving convictions in the last five years.
  • Criminal background check and Class I Fingerprint Clearance Card within 90 calendar days of employment.
  • Substance abuse testing upon employment and random testing during employment.
  • Updated immunization record and current Tuberculosis (TB) skin report upon employment.
  • First Aid/CPR Card within 90 calendar days of employment.
