Job Title: Physical Therapist - SNF / LTC

Centra Healthcare Allied is seeking a skilled Physical Therapist to join our team in Brooksville, Florida.

Job Summary:

We are looking for a highly motivated and experienced Physical Therapist to work in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) setting. The ideal candidate will have a strong background in physical therapy and excellent communication skills.

Responsibilities:
  • Provide physical therapy services to patients in a SNF / LTC setting
  • Develop and implement individualized treatment plans to meet patient needs
  • Conduct thorough assessments and evaluations to identify patient goals and objectives
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • 1 year of recent experience as a Physical Therapist in a SNF / LTC setting
  • Current Physical Therapist license in the state of Florida
