About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Aultman Health Foundation. As a Home Care Specialist, you will provide high-quality patient care and services on an intermittent basis to patients in their homes.

Key Responsibilities
  • Collect and Document Patient Data: Gather and record objective and subjective assessment data and observations about the patient's condition, and report response to treatments and changes in the patient's physical or emotional condition to the physician and RN.
  • Implement Care Plans: Carry out the current plan of care at the direction of the RN, providing nursing services, treatments, and preventive and rehabilitative procedures as assigned.
  • Document Patient Progress: Accurately and completely document notes of observations, treatments, and care of patients, and document the patient's response to the plan of care.
  • Collaborate with Interdisciplinary Teams: Contribute to the revision of the nursing plan of care, and participate in case conference meetings to discuss the need for involvement of other members of the interdisciplinary team.
  • Administer Medications and Treatments: Administer medications and treatments according to the Ohio Board of Nursing Nurse Practice Act.
  • Meet Productivity Standards: Meet daily average productivity standards established by the organization.
  • Display a Positive Image: Display a positive and professional image and attitude in all relationships with patients, families, peers, and in the community.
Qualifications
  • Education and Licensure: Graduate of an accredited school of practical nursing, and current State of OH LPN license.
  • Experience: Two years recent nursing experience in acute, long-term care, or home care, with at least one in home care preferred.
  • Skills and Certifications: Must be able to work independently, have demonstrated patient teaching skills, and strong written and verbal communication skills. Current BLS certification and valid OH Driver's License required.
Working Conditions
  • Hours and Shifts: Hours of operation with shifts as assigned.
  • Physical Demands: Subject to frequent interruptions and changes in priority of duties throughout the day. Subject to emergency and other crisis situations. Hazardous Exposure Category 1.
