Job SummaryWe are seeking a skilled Registered Dietitian to join our team as a Renal Nutrition Specialist. In this role, you will work closely with our interdisciplinary team to provide comprehensive nutrition care to patients with end-stage renal disease. Your expertise will be essential in helping us achieve our mission of delivering exceptional patient care and improving health outcomes.
Key Responsibilities- Assess patient knowledge and provide education on the relationship between diet and kidney disease.
- Develop and implement individualized nutrition plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to identify and address malnutrition and other nutrition-related issues.
- Monitor and document patient progress, making adjustments to nutrition plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Registered Dietitian (RD) certification.
- 1+ year of experience in clinical nutrition, preferably in renal care.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
