Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our Home Health team at AccentCare, Inc. As a Home Health Nurse, you will provide high-quality patient care and support to individuals in the comfort of their own homes.

Key Responsibilities:

  • Provide nursing visits to patients in their homes, following treatment plans prescribed by physicians
  • Conduct thorough assessments of patients' medical conditions and develop individualized care plans
  • Administer medications, perform wound care, and provide other medical treatments as needed
  • Maintain accurate and complete medical records, including documentation of patient interactions and treatment outcomes
  • Collaborate with healthcare teams to ensure seamless care coordination and communication

Requirements:

  • One year of clinical experience and one year of home health experience
  • Ability to provide nursing visits according to treatment plans and maintain accurate medical records
  • Graduate of an accredited practical or vocational nursing program
  • Currently licensed LPN or LVN in the state of agency operation
  • Valid CPR certification
  • Licensed driver with ability to travel to all business locations
