This role involves delivering exceptional care to adults diagnosed with mental health or substance use disorders. Reporting directly to the Director of Nursing, your responsibilities will include:
- Monitoring client health metrics, including vital signs and progress on treatment plans.
- Gathering both objective and subjective data from clients, and communicating any changes in their condition to the RN or healthcare provider.
- Administering injections, collecting samples for laboratory analysis, and documenting any adverse reactions to treatments as per established protocols.
- Utilizing your knowledge of medications, including psychotropic drugs, to educate clients and staff about health care needs.
- Identifying and addressing symptoms related to substance use and mental health conditions.
- Conducting screenings for substance toxicity and assessing risks to client safety.
- Documenting all findings accurately and consulting with the medical and clinical teams as necessary.
- Maintaining a safe, clean, and welcoming environment for clients.
- Ensuring thorough and precise documentation within the electronic care record (ECR) system and other required forms.
- Collaborating with the interdisciplinary treatment team and external agencies involved in client care.
- Coordinating client care activities, including appointment scheduling and treatment arrangements.
- Engaging in mandatory training, quality assurance initiatives, and continuing education for licensure maintenance.
Required:
- High School Diploma or equivalent.
- Current and valid LPN licensure in Ohio.
- A minimum of 2 years of experience as a Licensed Practical Nurse.
- Proficiency in electronic care record (ECR) systems and healthcare technology.
Preferred:
- Experience in behavioral health settings.
- Knowledge of confidentiality standards and nursing best practices.
- Strong interpersonal skills for effective communication with clients and colleagues.
- Excellent organizational and time management abilities.
Monday - Friday, 8:30 AM - 5:00 PM
