Baystate Health is seeking a skilled Social Worker to join our team in providing exceptional care to patients and their families. As a Social Worker - Hospice Care Specialist, you will be responsible for advocating for patients and their families, addressing psychosocial issues, and coordinating care to ensure optimal outcomes.
Key Responsibilities- Conduct thorough assessments of patients and their families to identify psychosocial issues and develop plans to address them.
- Provide counseling and support to patients and their families to help them cope with challenges and adjustments related to diagnosis and treatment.
- Coordinate care with healthcare professionals, patients, and families to ensure continuity of care and optimal outcomes.
- Develop and maintain relationships with community agencies and resources to provide patients and families with access to necessary services.
- Enter data and information into electronic medical records and other systems to maintain accurate clinical records.
- Prepare reports and maintain accurate records for legal action.
- Refer patients to appropriate community resources, including domestic violence referrals.
- Develop and maintain working relationships with community agencies and resources.
- Attend meetings with healthcare teams and network in the community to enhance care coordination and access to services.
- Disseminate meeting minutes and updates to leadership, staff, and teams regarding information and changes.
- Perform other job-related duties as assigned by the Practice/Department Manager or Senior Leadership.
- Master of Social Work degree from an accredited institution.
- Minimum one year of experience as a practicing social worker in a healthcare or social service setting.
- Equivalent experience in a Master's level healthcare setting internship may be considered.
- Preferred experience in an Ambulatory healthcare setting; internship experience will be considered.
- Spanish, Bilingual language skills are optional.
- Knowledge of local community services, local community resources, state, and federal programs that provide medical care and financial support to children and families.
- Ability to work independently and collaboratively in group situations.
- Demonstrated ability to be a team player.
- Excellent verbal and written communication skills in English.
- Ability to work with diverse patient and staff populations.
- Proficient in basic computer skills and Microsoft Office.
- Ability to navigate and document in clinical computerized patient care records.
