Oncology Social Worker Position

Job Summary:

We are seeking a Licensed Certified Social Worker (LCSW) to join our Ambulatory Oncology Team at Trinity Health. The successful candidate will support our locations at Mercy Medical Center and Radiation Oncology in Enfield, CT.

Key Responsibilities:

  • Assess and counsel cancer patients and families referred to the Cancer Center.
  • Provide consultation to physicians and staff in ambulatory discharge planning, from treatment to survivorship services.
  • Offer patient-centered counseling and consultative services based on individual needs assessments.
  • Communicate patient status information through written documentation and verbal discussions to ensure the team understands patient circumstances relative to achieving treatment plan and survivorship program goals.

Requirements:

  • Master's degree required.
  • Valid social work license from the Commonwealth of Massachusetts, LCSW required.
  • Valid Social Worker License from the state of CT, LMSW required.
  • Two years of related professional experience required.
  • Ability to communicate effectively both orally and in writing, including explaining clinical procedures to clients and their families, counseling clients and their families, and enforcing policies and regulations in difficult situations.
  • Work requires analytical ability to collect information from diverse sources, apply professional principles in performing various analyses, and summarize information and data to solve problems or design complex systems and programs.
